Nurse Injector – TRAIN TO HIRE

Fox River MedSpa is excited to offer their very first TRAIN TO HIRE Nurse Injector! A rare opportunity to receive paid training as an Aesthetic Injector, this position has very specific schedule requirements that the candidate must adhere to.

Team interviews start on Tues May 12th at 9:15, 9:30 or 9:45. You will have 10 minutes with each leader of our team, each of us asking different questions to get to know you, your current skill set, and your approach to this position. Plan on an hour. We will interview 15 candidates at this time.

After our round-robin interviews, we will invite up to 5 candidates to shadow with Nurse Injector Kari for 3 hours on 4 different shifts. You must have some availability on these days/times between May 18th-29th

 Mon 9am – 4pm
 Wed 10am – 3pm
 Thurs 9am – 4pm
 Friday 9am – 4pm

After the shadow shifts are complete, we will make our hiring decision.

The successful candidate will go under contract with us for Bootcamp + 12 months of service.

Our 60 shift Bootcamp (about 90 days, depending on schedule), paid at $30/hour, puts you alongside our nurse injectors to learn a solid foundation of aesthetic injecting. You will also help with GLP and IV guests as needed during this time. You must have availability that lines up with our current Nurse Injectors, including the occasional Saturday, during this time.

After Bootcamp, you will have a set schedule with Fox River MedSpa that must include 3 evenings and/or weekends per week. Rate of pay jumps to $36/hour plus 10% commissions, tips, and quarterly bonus pool for hitting retail sales targets. This schedule requirement is effective for 12 months.

If for any reason you separate early, there is a $15,000 training reimbursement fee.

AESTHETIC NURSE INJECTOR TRAINING-TO-HIRE AGREEMENT
Fox River MedSpa, LLC – Algonquin, Illinois
This Aesthetic Nurse Injector Training-to-Hire Agreement (“Agreement”) is entered into as of __________ (“Effective Date”), by and between Fox River MedSpa, LLC, an Illinois limited liability company (“Company”), and ________________________ (“Nurse”).

1. PURPOSE
This Agreement establishes the terms under which Nurse will participate in a structured training program with the intent of transitioning into an Aesthetic Nurse Injector role with the Company.

2. AT-WILL RELATIONSHIP
Nurse’s relationship with the Company is at-will, meaning either party may terminate the relationship at any time, with or without cause or notice, subject to the obligations contained in this Agreement, including the Training Fee repayment obligation.

3. INITIAL EVALUATION PERIOD (FIRST 2 WEEKS)
• Nurse will complete a mandatory two (2) week shadowing and evaluation period.
• Company retains sole discretion to determine continuation.
• Nurse will:
Follow a strict shadow schedule
Observe and assist with aesthetic procedures including neurotoxins, dermal fillers, PRP, and microneedling
• Company may terminate Nurse’s participation at any time during this period.

4. TRAINING CONTRACT PHASE (POST-EVALUATION)
Upon successful completion of the Evaluation Period:
• Nurse may transition into a training contract role at $30.00 per hour
• Nurse will:
Perform GLP injections and IV therapy services within scope
Continue shadowing all aesthetic procedures including neurotoxin, filler, PRP, and microneedling
Obtain Neurotoxin & Filler Certification if haven’t already, on their own expense
• Advancement remains at the sole discretion of the Company.

5. NINETY (90) DAY TRAINING PERIOD
• Nurse will complete a 90-day training and development period
• Company may:
Extend training
Modify responsibilities
Terminate based on performance or business needs
• Completion is determined solely by the Company, and Nurse trainee will be qualified for aesthetic services as skills progress.

6. POST-TRAINING COMPENSATION & SCHEDULING REQUIREMENTS
Upon successful completion of training:
• Compensation:
$36.00 per hour
10% commission on aesthetic services and retail sales attributable to Nurse
• Nurse must maintain:
Minimum 30 hours per week
At least three (3) evening and/or weekend shifts per week (11a-7p week nights and/or 9-2 weekends)
• Scheduling is determined exclusively by Company and may be adjusted at any time

7. TRAINING INVESTMENT & TRAINING FEE REPAYMENT OBLIGATION
A. Training Investment
Nurse acknowledges that Company provides specialized, hands-on aesthetic training, clinical supervision, proprietary techniques, and business education (the “Training”), which requires a substantial investment of time, resources, and expense by the Company.
B. Consideration for Training
In exchange for receiving this Training, Nurse agrees to remain actively employed with the Company for a minimum of fifteen (15) months following completion of the Training Period, with 30 hours per week including 3 evening and/or weekend shifts. (“Commitment Period”).
C. Training Fee Obligation
If Nurse voluntarily resigns, abandons position, or is terminated for cause prior to completion of the Commitment Period, Nurse agrees to reimburse Company a Training Fee in the amount of Fifteen Thousand Dollars ($15,000.00).
D. Nature of Training Fee
Nurse agrees that:
The Training Fee represents a reasonable reimbursement for training, onboarding, lost productivity, and business investment

  • The amount is not a penalty, but a fair estimate of costs incurred by Company
  • The value of the Training is not easily quantifiable with precision

E. Payment Terms

  • The Training Fee shall be due within thirty (30) days of separation
  • Company may deduct any allowable amounts from final compensation to the extent permitted by law
  • Any unpaid balance may be referred to collections or legal action

F. Enforcement Costs
Nurse agrees to be responsible for all reasonable costs incurred by Company in enforcing this provision, including attorney’s fees, court costs, and collection expenses.
8. CONFIDENTIALITY & PROPRIETARY INFORMATION
Nurse agrees to maintain strict confidentiality of all non-public Company information, including:

  • Client identities and treatment history
  • Pricing, protocols, and treatment methodologies
  • Training materials and techniques
  • Marketing strategies and business operations

This obligation survives termination.
9. NON-COMPETE (DURING EMPLOYMENT)
Nurse agrees that during the term of employment or engagement with Company, Nurse shall not engage in, work for, consult with, or provide services to any other business offering aesthetic, cosmetic, injectable, IV therapy, or similar services, whether paid or unpaid, without prior written consent from Company.
Any violation of this provision shall constitute material breach of this Agreement.

10. NON-SOLICITATION
During employment and for twelve (12) months following termination, Nurse shall not, directly or indirectly:

  • Solicit or attempt to solicit any client of the Company
  • Divert or attempt to divert business from the Company
  • Recruit or induce any employee or contractor to leave the Company

11. COMPLIANCE WITH LAW & LICENSURE
Nurse agrees to:

  • Maintain all required licenses and certifications
  • Operate within Illinois scope of practice laws
  • Follow all Company clinical protocols and supervision requirements

12. CLASSIFICATION
Company reserves the right to classify Nurse as an employee or independent contractor in accordance with applicable law. All obligations under this Agreement apply regardless of classification.

13. GOVERNING LAW
This Agreement shall be governed by the laws of the State of Illinois.

14. SEVERABILITY
If any provision is deemed unenforceable, the remainder shall remain in full force and effect, and the provision shall be modified to the extent necessary to be enforceable.

15. ENTIRE AGREEMENT
This Agreement constitutes the entire agreement between the parties and supersedes all prior discussions. Any modifications must be in writing and signed by both parties.

16. ATTORNEY’S FEES
In any dispute arising from this Agreement, the prevailing party shall be entitled to recover reasonable attorney’s fees and costs.

17. ACKNOWLEDGMENT
Nurse acknowledges that:

  • They have read and understand this Agreement
  • They have had the opportunity to seek legal counsel
  • They voluntarily agree to all terms, including the Training Fee obligation
